Junior High (Grades 6-8): Additional Required Forms & Information
KSHSAA Sports Physical Form If your student will be participating in ANY sport, the student must have a completed KSHSAA (Kansas High School Activities Association) sports physical form, dated after May 1, of the current year, on file before they will be allowed to practice. 6th-8th grade students are advised to get their physical done during the summer. All pages 1-5 must be completed and signed.
KSHSAA Concussion Form You must complete the Jr. High concussion form each year before your student will be allowed to practice. Please attach this form to the Sports Physical. Both Student and Parent must sign the form.
Physical Education Shirts
Jr. High Physical Education classes are required to have a Luckey Jr. High P.E. shirt. If you would like to order shirts for your student, please fill out the jotform found in the enrollment information on the Home Page. Payment of $10 per shirt will be billed via Brightwheel.

Authorization for Dispensing Medication: (prescription or non-prescription)
If your child needs to have ANY medication-prescription (including inhalers or epi pens) or over the counter (Acetaminophen, Ibuprofen, cough drops etc.)- dispensed at school, this form needs to be on file in the office. All prescription medications must be in the original container with complete student name, date filled, and the name of the licensed physician who wrote the prescription

Medical Statement to Request Meal Modification (Food Allergies) Form:
If your child has a food allergy or needs dietary modifications because of a medical condition, disability, etc., this form must be signed by a "medical authority" and submitted each year to the lunchroom. Please read over the complete Food Modification Policy before completing the form.
Cafeteria - Lunch/Milk Payment: K-8th Grade Breakfast $1.70 & Lunch$3.00, Guest Breakfast $2.50 Lunch $3.85. There is a fee of $.70 for extra milk or milk for a student brings a home lunch. If you would like to make a payment please provide a check to MCS Lunch Room/ or cash for desired amount and send in with your student or drop off to the MCS Office. Breakfast and Lunch are available.
Band Instrument Maintenance Fee for MCS rentals, for students in 4th - 8th grade. For those students wanting to rent an instrument from MCS band program, check with the band teacher for the maintenance fee due upon receiving the instrument. More guidance from our Music Teacher will be provided during "Back to School Night."

- At least 2 MCS shirts (at least one red for All School Mass days) are recommended for starting the school year; more pieces can be purchased at the discretion of parents. Only certain items (shirts, jumpers, all logo pieces) are a purchase required from Land's End or our MCS Uniform Closet of gently used uniforms; other items (shorts/pants) can be purchased elsewhere. - Throughout the year Manhattan Catholic Schools hosts special fundraising events. These fundraisers support our operating budget, special projects, enhanced learning programs, and much more for MCS. - Manhattan Catholic Schools offers a Preschool program for 3 - 5 year olds. The program is designed to provide a safe, professional and quality accredited educational program. Our developmentally appropriate curriculum within the MCS Christian environment includes all areas of growth: religious, cognitive, language, physical, emotional and creativity.

- Manhattan Catholic Schools helps working families by providing an excellent, onsite After School Program. Students between grades Kindergarten - age 12 are able to attend Cardinals Club in two State licensed classrooms. -- WAITLIST FOR 21-22 School year

- Manhattan Catholic Schools PTO is a source of secondary support to the school. There are countless ways to volunteer, meet other parents, support your child's classroom experience, and financially support MCS through the PTO.
- Booster Club supports the MCS athletics and Scholars Bowl teams. Volunteers support the Booster Club through concession stands at home volleyball, football, and basketball games. MCS Booster Club also serves as the spirit coordinators for athletes -- decorating lockers, having a watermelon feed, and cheering on the teams to victory!

Admissions Information
Thank you for considering Manhattan Catholic Schools. Our mission is to be a faith community working in partnership with parents, and inspired by the Holy Spirit to develop the whole child — mind, body, and spirit — in accordance with our Catholic faith traditions.
Steps for New Student Admission
Initial Application – Please complete a Pre-Registration form through the jotform link on the home page. After submission you will be invoiced through Brightwheel from our finance department for $100 (per child) that is a non-refundable application fee.
Interview – You may contact the office to arrange a time for an interview with the principal, child, and parents to mutually ascertain if the school is a good fit for the child and family. The principal may contact principals or counselors at previous schools attended.
Final Steps – At the noted date (usually in July) enrollment links to the online jotforms will be posted on the homepage for annual paperwork for new and returning students including record release forms for new students so we can obtain cumulative records from previous schools. Each new student admitted is subject to a 3-month probationary period. A payment schedule is arranged.
Required Records for Admission
- Proof of immunizations. Either a shot record or completed Kansas Certificate of Immunization (KCI green form) is necessary. Requirements can be found at https://www.kdhe.ks.gov/324/Immunization-Requirements
- A Health Assessment performed within the 12 months prior to entering completes the required medical procedures for newly entering students. Parents may also choose to include hearing and vision screenings
- A state issued Birth Certificate (Copy)
- Baptismal and other applicable Sacrament certificates (Copy) (for Catholic students)
Tuition & Financial Aid
Manhattan Catholic Schools has contracted TADS to determine financial aid awards to those families that apply for tuition scholarship. Applications are made online with TADS and financial information requested will be sent to them. When all information is complete, they will calculate the scholarship awards and notify recipients. You will have access to TADS with login information to follow the process. (Please contact the Principal for more information). When all applications are completed and calculations made by TADS, they will send families notification of aid we can give. They have supplied a worksheet to use but this will never be sent to TADS and is for your use only. Select the application tab to send completed information to TADS.
MCS has a benefactor for financial aid. However the amount cannot meet the needs of all of our families. Thus, no family can receive more than 50% of the cost of tuition for their family. Other fees must be paid for by the family. MCS pays an annual cost for the program. The application fee for families is $42. Thank you for understanding the change in our process.
